Terms of Service

These Terms of Service govern access to and use of Chimera Forms, including accounts, form creation, hosted forms, embeds, collaboration, email features, scheduling, response management, exports, and related services. By creating an account or using Chimera Forms, you agree to these terms.

1. About Chimera Forms

Chimera Forms is a standalone form, survey, registration, scheduling, and workflow platform. It allows authorized users to create and publish forms, collect responses, collaborate with other users, manage capacity and availability, send notifications, and work with collected data.

In these terms, operator means the person or organization making this Chimera Forms service available. Form owner means the user or organization responsible for a form. Respondent means a person who submits information through a form.

2. Accounts and access

You are responsible for providing accurate account information, keeping your credentials secure, and using only accounts and forms you are authorized to access. Do not share passwords or attempt to bypass account roles, collaboration permissions, or other access controls.

If you believe your account has been compromised, notify the operator promptly and change your password as soon as practical.

3. Creating and publishing forms

Form owners are responsible for the forms they create, including questions, instructions, branding, logic, email triggers, capacity settings, custom CSS, hosted-page content, and any information requested from respondents.

You must make sure your forms are lawful, appropriate for their intended audience, and consistent with any policies, contracts, professional requirements, research requirements, consent obligations, or regulations that apply to your work.

4. Your content

You retain responsibility for and rights to the content you place in Chimera Forms. You grant the operator the limited permission necessary to store, process, render, transmit, back up, and otherwise handle that content for the purpose of providing and securing the service.

You may not use Chimera Forms to publish content that is unlawful, malicious, deceptive, defamatory, infringing, abusive, or designed to compromise another person or system.

5. Form responses and collected data

Form owners determine what information their forms collect and are responsible for having an appropriate reason and, where required, permission to collect it. Collect only the information you reasonably need.

Form owners and authorized collaborators are responsible for protecting response data they can view or export. If a form collects personal, confidential, regulated, educational, research, financial, health-related, or other sensitive information, the form owner is responsible for determining whether Chimera Forms is appropriate for that use and for meeting the requirements that apply to the project.

6. Collaborators and permissions

Form owners may invite other people to collaborate and may assign different access levels. The person granting access is responsible for choosing an appropriate role and removing access when it is no longer needed.

Invitation links and account activation links are intended only for the recipient. Do not forward, publish, or misuse invitation links.

7. Email notifications and invitations

Chimera Forms may send account invitations, collaborator invitations, confirmation messages, form notifications, and other messages configured by authorized users. You are responsible for making sure the recipients and content of messages you configure are appropriate and lawful.

Do not use Chimera Forms for unsolicited bulk messaging, spam, phishing, harassment, or attempts to disguise the origin or purpose of a message.

8. Hosted forms and embeds

Forms may be shared through a hosted link, QR code, or JavaScript embed on another website. Form owners are responsible for where they publish those links or embeds and for any surrounding website content, notices, or consent language required for their use.

Public form access does not grant administrative access to the form, its responses, or the Chimera Forms account that created it.

9. Scheduling, quotas, and capacity

Chimera Forms includes date rules, quotas, and capacity controls that can help manage registrations, tours, appointments, events, and similar activities. Form owners are responsible for configuring these rules correctly and reviewing collected responses when capacity or availability is operationally important.

Chimera Forms is designed to enforce configured limits server-side, but no software system can guarantee that external circumstances, human error, connectivity failures, or incorrect configuration will never affect scheduling or availability.

10. Acceptable use

You may not use Chimera Forms to:

  • gain unauthorized access to accounts, forms, responses, or systems;
  • probe, scan, attack, overload, or interfere with the service without permission;
  • distribute malware, malicious code, phishing content, or deceptive forms;
  • harass, threaten, impersonate, or unlawfully profile another person;
  • collect information you are not legally or ethically permitted to collect;
  • circumvent rate limits, security controls, quotas, permissions, or abuse protections;
  • use the service in a way that violates applicable law or the rights of others.

11. Exports and external systems

Authorized users may export response data and may configure features that send information by email or to other systems. Once information leaves Chimera Forms, the person or organization receiving or storing that information is responsible for protecting it appropriately.

Third-party services may have their own terms, privacy practices, availability, and security requirements. Chimera Forms is not responsible for services operated by third parties.

12. Security

Chimera Forms includes security controls intended to protect accounts, forms, and responses. You agree not to defeat or interfere with those controls. If you discover a possible security issue, report it privately to the operator rather than exploiting it or publicly disclosing sensitive details before the issue can be addressed.

13. Availability and changes

Chimera Forms may change over time as features are added, bugs are fixed, security is improved, or workflows are refined. The operator may perform maintenance, restrict access to a feature, or temporarily interrupt service when reasonably necessary.

No online service can promise uninterrupted or error-free operation at all times. Form owners should plan appropriately when a form supports a time-sensitive or mission-critical activity.

14. Suspension and termination

The operator may suspend or disable accounts, forms, invitations, embeds, or access when reasonably necessary to protect users, data, the service, or legal compliance. Examples include abuse, unauthorized access, malicious activity, repeated security violations, or prohibited use.

15. Disclaimers

Chimera Forms is provided as a general-purpose form and workflow platform. It is not legal, medical, financial, compliance, or research-ethics advice. You are responsible for deciding whether a particular form, workflow, data collection practice, or retention practice is suitable for your needs.

16. Limitation of responsibility

To the extent permitted by applicable law, the operator is not responsible for indirect or consequential losses arising from user-created forms, incorrectly configured logic or capacity rules, unauthorized sharing by users, third-party services, or data exported from Chimera Forms. Nothing in these terms excludes rights or responsibilities that cannot legally be excluded.

17. Changes to these terms

These terms may be updated as Chimera Forms evolves or as legal and operational requirements change. The date at the top of this page will be updated when material revisions are made. Continued use of the service after revised terms take effect may constitute acceptance where permitted by law.

Questions about these terms should be directed to the operator of this Chimera Forms service. Questions about the content or purpose of a particular form should normally be directed to the person or organization that published that form.
